Building a neighbourhood health service and expanding primary and community services

The HSJ Care Closer to Home Forum will bring together 120+ senior decision-makers, including Medical Directors and Strategic commissioning leads from PCNs, CICs, VCSEs, Adult Social Services and Community and Mental Health Trusts to explore solutions such as urgent care, preventive measures, and integrated services. This event offers a platform to share strategies that strengthen care outside of hospitals.

Plenary sessions will cover the overarching themes that concern all practitioners and decision-makers:

  • Redesigning care pathways for enhanced community services
  • Strengthening preventive care to keep patients out of hospitals
  • Fostering collaboration for integrated care delivery
  • Developing proactive strategies for admission avoidance
  • The role of urgent care in alleviating acute pressures
  • Implementing care closer to home models
